October Reminders Including Our Free Onsite Flu Vaccine Clinic and Follow-Up Information from our COVID-19 Vaccine Resource Presentation

Dear Families,

It was a pleasure to see many of you at Back to School Night last week.  Today we have follow up information to share as well as calendar reminders for the month of October.

1) COVID-19 Vaccine Resources: 

  • Assessing Families’ Interest in a Potential Onsite COVID-19 Vaccine Clinic:   Our School Nurse, Lyn Ross, is happy to serve as a resource anytime for answering individual questions from families about COVID-19 vaccines or assisting families with coordinating vaccine appointments in your local communities.  In addition, Milestones would like to assess the level of interest from families in potentially offering an onsite COVID-19 Vaccine clinic if we have sufficient participation.  Please email Lyn Ross: lross@advancingmilestones.com if you would be interested in registering your Milestones student(s) for an onsite clinic, if one is offered. Parents, guardians, and siblings would be welcome to participate along with your Milestones student(s), and you can specify your anticipated number of participants.  This inquiry is open both to families with students who are 12+ and are currently eligible and for families with students under the age of 12 who are anticipating upcoming eligibility once a vaccine for ages 5-11 is available.

2) October Calendar Reminders:

  • Onsite Flu Clinic Sign-Up/Forms Deadline:  Extended to Friday, October 8th
  • Half-Day (12:00pm Dismissal for Students and Faculty):  Friday, October 8th
  • No School For Holiday Observance:  Monday, October 11th
  • Onsite Flu Clinic at Milestones For Registered Faculty and Students:  Tuesday, October 12th
  • Half-Day for Professional Development (12:00pm Dismissal for Students): Wednesday, October 27th
  • Halloween Activities with the Option to Wear Costumes (*Note: more information is forthcoming this month from case managers): Friday, October 29th  

3) Free Flu Vaccine Clinic Reminder:

Please see below for an opportunity to have your student(s) participate in our free onsite flu vaccination clinic that is open to all students.  We have attached a copy of the required consent form with information about the flu vaccine to this email.

We are pleased to announce that we will again be offering the Free Onsite Flu Shot Clinic to All Students and All Staff.

Date: Tuesday, October 12, 2021 from 10:00AM – 1:00PM 

Where: Onsite at Milestones

Requirements: 

  1. Sign Up for A Time Slot

  2. Provide Copy of Health Insurance Card

  3. Provide Signed Consent Form (download here) (signed by a parent/guardian if the student is under 18; signed by the student if the student is over 18)

  4. IMPORTANT: If you would like your student to participate, please email or turn in hard copies of the required consent form and a copy of the medical insurance card to our School Nurse, Lyn Ross: LRoss@advancingmilestones.com OR Your Student’s Case Manager.

* All forms are due by no later than Friday, October 8th if you would like your student to participate in the onsite clinic.
